If your account has been locked due to too many unsuccessful login attempts, you will see a message that says "Too many incorrect attempts. Check your email for unlock instructions" at the login screen
To unlock your account check your inbox and click on the email from support@fulcrumapp.com. Please check your spam folder if you do not see it in your main inbox.
Then click on the "Unlock my account" link.
